Noble.MD Introduces Population Health Reports for its Theo Platform
Dr. Todd Johnson, CEO on The Entrepreneur's Network
Dr. Johnson discusses how Theo, the 1st Digital Healthcare Advisor, is revolutionizing Population Health by identifying patient health risks at the time-of-care and activating them to change behaviors through enrollment in health programs in real-time.
Noble.MD unveils Theo 2.0
Based upon finding from extensive user testing, we've improved Theo to provide a more engaging interactive user experience for both patients and clinicians. Theo 2.0 brings more color and fun to the patient, while providing an extremely workflow-friendly and informative source of patient data to clinicians.
This revolutionary new mobile health application takes patients through simple health lessons that provide critical learning while assessing risks relating to standardized health topics and intended adherence to treatments. Designed to fit seamlessly into clinicians’ time-constrained workflow, Theo Knowledge Prescriptions average just five minutes and are delivered while the patient is waiting in the exam room for their consultation.
Theo facilitates a clear flow of information among patients, clinicians, and healthcare providers. Theo assists clinicians in engaging patients on even the most difficult and uncomfortable topics, allowing for open, informed shared decision-making. In studies of socially challenging topics, Theo has been shown to result in a dramatic increase in openness of clinician-patient-parent communications.
With this latest version of Theo, Noble.MD is reinventing patient engagement at the point-of-care. Theo is Interactive Population Health Management.
The Children's Hospital of Philadelphia
Noble.MD is partnering with The Children’s Hospital of Philadelphia (CHOP) on a new approach to increasing HPV vaccination rates in adolescents. Despite CDC recommendations that every child be vaccinated at age 11 or 12, less than 33% of girls and 15% of boys are receiving the full 3-dose regimen of this potentially life saving vaccination. Developed jointly with vaccination education experts at CHOP with the goal of increasing the frequency of vaccination recommendations by clinicians, Theo™ simultaneously educates patients and parents about the importance of vaccination to prevent HPV-related diseases.
Chen Neighborhood Medical Centers, a leading ACO in Southern Florida providing care to over 10,000 Medicare Advantage patients, has collaborated with Noble.MD on a new approach to engaging seniors on the benefits of annual influenza vaccination. In 2013, Theo™ produced a 22% increase in vaccination rates among those patients receiving the DHI. This engagement also confirmed Theo’s™ ease of use and understanding among elderly and Spanish speaking patients.
JenCare, a premier Medicare Advantage ACO with locations throughout six states, is partnering with Noble.MD to engage their patients in key health topics relative to senior populations: Advance Directives and Fall Prevention. Early results from a fall prevention study found that 22% of patients who had fallen had not been willing to talk to their clinicians were willing to report the information to Theo™. This improved clinician-patient interactions and increased enrollment in a provider-sponsored fall prevention initiative. Results from the Living Will DHI show that clinicians using Theo™ to engage patients produce 8x the number of Living Wills than those who do not.
